Entries will only be accepted through appointments made with our clinic by December 31, 2024. Winner must be 18 years of age or older. Winner must agree to pick up their prize at the clinic. Prize will not be mailed. Employees of the practice and immediate family members are not eligible to participate. Winner may not request substitution or other consideration from the originally awarded prize. These rules are subject to change without notice.
